Getting started with care at Health Solutions is simple. Whether you’re new to our services or returning for treatment, our Open Access program makes it easy to connect with the support you need. Just call or visit one of our locations, and our team will guide you through the next steps toward improved health and well-being.
During your Open Access appointment, you’ll receive a comprehensive assessment and personalized recommendations to help determine the services that best meet your unique needs.
The locations listed below provide Open Access enrollment and new client services for both youth and adults.
Open Access in Pueblo, Walsenburg, & Trinidad
In Pueblo:
Available by tele-video or in-person Monday – Friday from 8am to 3:30pm
- Youth and Family Services – 1012 West Abriendo Avenue, Pueblo, CO 81004
- Crestone Recovery – 41 Montebello Road – Lower Level, Pueblo, CO 81001
In Walsenburg and Trinidad:
Available by tele-video or in-person Monday – Friday from 8am to 11am and 1pm to 3:30pm
- Trinidad location – 417 South Indiana Avenue, Trinidad, CO 81082
- Walsenburg location – 926 Russell Street, Walsenburg, CO 81089
Our Family Medicine and Residential Programs will continue to serve Pueblo and the surrounding communities by phone appointment.
