About the CARE and CARE Youth Teams
The CARE and CARE Youth teams provide intensive, community-based behavioral health services for individuals living with serious mental illness who need more support than traditional outpatient treatment can provide. CARE follows the Assertive Community Treatment (ACT) model, which is the term commonly used in Colorado and nationally for this type of multidisciplinary, community-based service. CARE Youth provides similar intensive services tailored to the developmental, family, educational, and social needs of adolescents and young adults.
The purpose of CARE is to help individuals remain stable and successful in their communities while reducing psychiatric hospitalizations, emergency interventions, homelessness, legal involvement, and other disruptions associated with serious mental illness. Rather than relying primarily on office-based appointments, CARE staff frequently provide services in clients’ homes and other community settings. This allows the team to identify barriers as they occur and provide practical, individualized support.
CARE uses a multidisciplinary team approach, meaning clients receive support from a team of professionals rather than relying on a single provider. Services may include psychiatric evaluation and medication management, medication support and delivery, individual therapy, case management, peer support, crisis intervention, care coordination, transportation related to treatment, assistance accessing community resources, and development of independent living skills. The team also works with clients on goals involving housing, education, employment, relationships, physical health, and greater independence.
Clients may also participate in groups and community-based activities designed to build practical skills, increase social connection, and support recovery. Some groups are led or supported by peer specialists and may focus on communication, coping skills, healthy routines, self-care, community navigation, social skills, and independent living. Recreational and community activities provide opportunities for clients to practice skills, build relationships, decrease isolation, and have positive experiences in the community.
CARE Youth provides many of these same services while placing additional emphasis on family involvement, school functioning, social development, community safety, age-appropriate independence, and preparation for adulthood. The team collaborates with parents or guardians, schools, treatment providers, human services, and other supports when appropriate.
CARE and CARE Youth serve individuals whose serious behavioral health needs significantly affect their ability to function successfully in the community. Clients may have experienced repeated psychiatric hospitalizations or crises, difficulty consistently participating in traditional outpatient treatment, medication adherence concerns, housing instability, safety concerns, or involvement with multiple treatment, legal, educational, or social service systems.
Referrals may be made by hospitals, behavioral health providers, residential programs, schools, courts, human services, community agencies, or other professionals involved in an individual’s care. Community members can submit referrals to CAREreferrals@health.solutions. Each referral is reviewed to determine eligibility and whether the program is an appropriate level of service for the individual’s needs.
The overall goal of CARE and CARE Youth is recovery, stability, and increasing independence. Through consistent outreach, individualized treatment, practical assistance, skill development, and community-based support, the teams help clients remain engaged in treatment, strengthen daily living and social skills, reduce crises and hospitalizations, and develop the skills and confidence needed to live as independently and successfully as possible.
