Connecting individuals to the right support before crisis develops

Health Solutions’ Mobile Early Intervention Services (MEIS) team members assist people in need to determine choices they have, reinforcing coping skills, and provide connections to support networks.

Our mobile team provides a face-to-face response to wherever the need is present. This program emphasizes wellness and recovery principals that assist those in need through difficult situations.

Our mobile clinicians will assist you in deciding the best way to manage your situation and help you access appropriate treatment.

OUR SERVICES INCLUDE:

  • Helping clients identify and use effective coping strategies
  • Connecting individuals to medical, behavioral health, and community resources
  • Providing early intervention during times of stress, trauma, or life changes to prevent escalation into crisis
  • Re-engaging individuals who have become disconnected from services or are experiencing decompensation
  • Offering motivation, information, and hands-on assistance to help clients engage in longer-term treatment
  • Delivering brief therapeutic support, check-ins, and resource navigation to stabilize situations

TO REFER, PLEASE INCLUDE THE FOLLOWING INFO:

  • Client’s name, date of birth, phone number, and address
  • Insurance provider and ID number (if available)
  • Guardian/contact info (if applicable)
  • Reason for MEIS referral or specific outreach goals
  • Any known history of violence or aggression (please describe)
  •  Are there any firearms or weapons in the home? (If yes, describe)
  • Any other known safety risks? (If yes, describe)

*Please Note: MEIS is a mobile outreach service. We will attempt in-person contact before making phone calls. If a phone call is preferred before a home visit, please clearly indicate that in the referral.
