Perry Hall Fills Critical Gap in Pueblo Community

When Health Solutions opened the doors of Perry Hall in January 2025, it ushered in a new era of health in a part of the community that previously had little access to care. 

For Pueblo’s St. Charles Mesa community, behavioral and primary care services were minimal. Now, thanks to Perry Hall, the community has access to both and more, all on the same, state-of-the-art campus. 

“Perry Hall is filling a critical gap in care in Pueblo,” said Vice President of Medical Services Andrea Alvarez. “By embedding behavioral health providers in a primary care setting and adopting the Primary Care Behavioral Health (PCBH) model, we’ve created a truly integrated and accessible health home for individuals and families.”

Perry Hall is supported by a multidisciplinary team that includes primary care providers, behavioral health clinicians, nursing staff, medical assistants, care coordinators, front office staff, and resource specialists. Altogether, 15 staff members contribute to daily operations and patient care. 

The care doesn’t stop at Perry Hall. Local families also have access to services at the Family Resource Center, which is located on the same campus and helps connect Perry Hall patients to vital community resources. The Learning Tree is also located on campus. 

Why is it important to offer integrated care? The answer comes down to the strong connection between mental and physical health.  

“Many patients present with co-occurring physical and behavioral health needs, and our team-based approach helps break down barriers to care and reduce stigma,” Alvarez said. “In an area like Pueblo, where health disparities and access issues persist, integrated care is essential to improving overall community health.”

The impact of integrated care has been clear. Clinicians have noticed improved engagement in behavioral health services and increased follow-up on chronic condition management because of warm handoffs and collaborative care planning between providers. 

As the work of Perry Hall continues, its programs continue to grow. In August, the Perry Hall team launched the Youth Empowerment Program (YEP), which provides prevention, early intervention, and resiliency-focused services for youth. 

In addition, the team hosted the third annual free sports physicals event. More than 160 young athletes received free sports physicals during this vital service that removed financial and access-related barriers for families.
